Enrolling a New Student to DCHS

2019-20 Registration Forms - English

Information for 2019-2020

Please read the following information, which we hope will answer many of your questions. We look forward to assisting you in the Counseling Center.

Enrollments are by appointment only, after we have received all of the required documents and forms listed below.

1) Please provide copies of the following documents required for all enrollments:

  • Proof of Residence (one of the following)
    • Warranty Deed
    • Purchase Agreement(valid until closing of property, then we will need the Warranty Deed)
    • Deed of Trust
    • Lease/Rental Agreement
    • Property Tax Notice
    • Notarized Letter (if living w/another family in area)
      • with one of the above documents
  • Immunization Record
    • Coloradostate law requires that we must have this at the time of enrollment or the student will not be admitted
  • Birth Certificate
    • A copy of an original certified state birth certificate.
  • Unofficial High School Transcript
    • a MUST for upper classmen in grades 10-12 (Counselors must have it to help in credit check and scheduling classes.)
  • Withdrawal Form (from previous high school)
    • Must have current grades only if transferring during the school year
    • State law requires that your student must be withdrawn from the current school before we can enroll him/her
  • Completed, original DCHS Enrollment Forms 1-8, which can be printed using the purple buttons at left
    • For Enrollment Form 5: please read the information on pages 2-5 and print and bring in only the signature page (page 1)

Please submit all of the above documents to our office prior to making an enrollment appointment. The documents can be hand delivered to the Counseling Center located in the North Building.
